Consultation request form for life and health
A form that starts a conversation rather than an application. It asks what the visitor wants to cover, their zip, a rough age band, whether they currently hold a policy and the best time and way to reach them. It does not ask for health conditions, medications or income; those belong in your call, under your process. Requests land in your CRM with the product line tagged so you know the conversation before you dial.
A quote request is not a quote, an application or a binder. Nothing is in force until a licensed producer and a carrier bind it.

Questions from life and health agents
Should the site quote term life rates?
No, and we will not build it to. Rates depend on underwriting that only you and the carrier complete, and a number on a page becomes a promise the visitor holds you to. The site explains what affects a premium in general terms, shows what a consultation covers and asks for the conversation. Any illustration you produce for a client is delivered by you, not by the website.
Can I keep the site as a solo agent, or is this only for agencies?
Solo agents are the most common life and health client we build for. The Single Line plan carries every product page, the consultation form and the two researched articles a week, and the site is managed entirely by us, so there is nothing for you to maintain between appointments. The build fee is invoiced only after launch, so nothing is paid up front beyond the first month.
What kind of articles would you write for a life agent?
Plain explainers that answer the questions your prospects search before they call: how term and whole life differ, what happens to coverage when a job ends, how final expense plans work for families. Each is 2,500 to 3,200 words, cites its sources, and states clearly that it is general information rather than advice, then points readers to your consultation form.
Can the site handle Medicare as well?
Yes, if you write it, a Medicare section is added with its own pages and its own request form that asks for zip and Part A and B effective dates. Your disclaimer wording is placed where you tell us. Medicare marketing has its own rules and you remain responsible for that compliance; we build what you approve and change it as a revision when your requirements change.
